The Minnesota Department of Human Services (DHS), working with many others, helps people meet their basic needs so they can live in dignity and achieve their highest potential. DHS is sponsoring the EAS project using approved state and federal funds. Program staff in the Care Delivery and Payment Reform, Behavioral Health, and Continuing Care areas are helping to focus outreach and onboarding to providers who are participating in initiatives to improve health outcomes.
